SunState BodyWorks™ requires that each new client completes and signs an Intake Form prior to their appointment. Intake Forms are kept completely confidential in compliance with HIPAA Privacy standards.
By scheduling an appointment with SunState BodyWorks™ you agree to the following conditions:

I understand that massage therapists are not qualified to perform adjustments, diagnose, prescribe, or treat any physical or mental illness, and that nothing said in the course of the session given should be construed as such.
I understand that the massage should not be construed as a substitute for medical examination, diagnosis, or treatment and that I should see a physician or other qualified medical specialist for any mental or physical ailment that I am aware of.
Because massage should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions, and answered all questions honestly. If I experience any pain or discomfort during my session, I will immediately inform the therapist so that the pressure and/or strokes may be adjusted to my level of comfort.
If I am receiving a Hot Stone Massage Service, I agree that I will communicate to my Therapist if the temperature of the stones is uncomfortable for me or of any other necessary adjustments. Therapist shall not be held liable if the client fails to communicate with the Therapist directly. I understand that a Hot Stone Massage Service may leave temporary heat marks.
I further understand that SunState BodyWorks™ allows for the client to be not draped during services. I agree to keep a professional environment. Inappropriate misconduct will result in my session being terminated and payment in full will be required.
